I further acknowledge that I am solely responsible for my own safety and will take all necessary and reasonable steps to ensure not only my safety but that of fellow volunteers and QCM participants. I am aware that my involvement with the QCM as a volunteer could result in death, injury, lose or damage to my person or property and freely assure emergency care in the event of my illness or injury and waive all privacy requirements as they relate to the use of my recent photograph by the QCM in connection with the event.
